Notice of termination

The term of notice is one full calendar month for the tenant. The tenant pays rent during the term of notice. The term of notice starts from the last day of the month in which the tenancy is terminated; e.g., if the apartment is terminated on 15 June, the tenant's agreement ends on 31 July. The date of termination is the day the form is sent.

All fields marked with an asterisk (*) are required.
